For credit requests exceeding $5,000,000, attach a certified board resolution that:

Authorizes the company to enter into a credit relationship with VoltFMP, Inc. (DBA DCPacket) Identifies the authorized signers Specifies the maximum credit amount authorized Is dated within 12 months of this application

Authorization & Certification

By signing below, the Applicant:

Certifies that all information is true, complete, and correct.
Authorizes VoltFMP, Inc. (DBA DCPacket) to obtain credit reports from D&B, Experian Business, Equifax Business, and any references listed.
Authorizes sharing of credit experience with other credit grantors and agencies.
Agrees that credit terms are subject to DCPacket Standard Terms and Conditions of Sale.
Agrees to a finance charge of 1.5% per month (18% per annum) on past-due balances.
Agrees to be responsible for all collection costs including reasonable attorney fees upon default.
Agrees that VoltFMP, Inc. may file a UCC-1 Financing Statement or take other security interest.
Agrees that approved credit may be modified, reduced, or revoked at DCPacket’s sole discretion.
Acknowledges that approval does not guarantee any specific credit limit or payment terms.
